Only 30% of B12 blood serum tests are accurate; B12 < 300 total B12 means there is a deficiency . Functional deficiency can also be with 450. With symptoms of B12 deficiency. You need injections. MMA is only 60% accurate, ranges are very wide above 25 is sign of B12 deficiency. Homocysteine needs to be done immediate after taking blood sample. You want Homocysteine to be lower than < 8. GP's cannot order it. 

Auto immune conditions: very often malabsorption of B12.

SIBO.

MTHFR gene mutation, then methylates badly, always AVOID FOLIC ACID from food and supplements. Many people, 40% have a variant of the MTHFR gene.

Ferretinine should be no lower than 30 or better above 50. Folate should also not be too low, <10 around 20-45 nmol/L. Otherwise B12 injections do not work. Also plenty of D > 80nmol/l, Magnesium intake via supplements always, potassium (eat avocado a day), zinc selenium, iodine and molybdenum also enough copper to absorb the iron.
